Facility Fees
- This facility's fees cover the use of the facility only. The facility fees do not include laboratory, pathology, surgeon, anesthesiologist, or certified nurse anesthetist fees. You will be billed separately for these fees.
- If you require special financial arrangements, please phone our office prior to your surgery to discuss alternative methods of payment.

Collections and Payment Policy
- Be prepared to pay any co-pay or deductible amounts on the day of your surgery.
- Patients who do not have insurance coverage or those having cosmetic surgery are also required to pay their charges in advance.
- We will bill your primary and secondary insurance carriers or governmental agency directly for the facility's charges.
- This facility accepts cash, cashier's checks, credit cards, and personal checks with a valid I.D.
Payment for the patient's portion is preferred and expected when you check in for your surgery or pain management procedure.
Prior to your surgical procedure or pain management procedure, the staff at the Surgery Center of Pinehurst will verify your coverage and benefits with your insurance company. Our staff will contact you regarding your financial responsibility based on the information provided by your surgeon in regards to the procedures and the information obtained from your insurance company regarding your benefits.
Insurance
- We will bill your primary and secondary insurance carriers or governmental agency directly for the facility's charges.
- Be sure to bring your most current insurance, Medicare, or public assistance card with you on the day of your surgery. If you have more than one insurance carrier, we will also need accurate secondary billing information.
- Please be aware of any admission policies your insurance plan may have. You or your physician may have to adhere to certain requirements in order to insure maximum reimbursement. Failure to obtain pre-authorization, physician referral, or a second opinion may greatly reduce or eliminate your benefits.

If the Surgery Center of Pinehurst is not contracted with your insurance company...
The Surgery Center of Pinehurst honors the in network benefit on the patients plan for any carrier that we are not contracted with as long as the patients plan has both in and out of network benefits. Therefore, if we do not have a contract in place with your insurance company, we will accept payment in an "out of network" agreement where the patient will not pay any more than they would have if we were contracted with the insurance company. We base the amount you will owe on your in network co-pay, co-insurance and out of pocket maximums.
Medicare
Medicare beneficiaries are welcome to receive services at this facility. This facility is certified by Medicare and complies with specified physical plant, staffing, safety and governance standards.

Facility fees for approved services at an Ambulatory Surgery Center (facility where surgical procedures are performed, and the patient is released the same day). You pay co-insurance, and Part B deductible applies.
